“I think osteopathy is really good, but I can’t explain it to anyone.”
Many patients tell us this. They want to recommend OQ to friends or family. But they struggle to describe what actually happens during a session.
It makes sense. You lie on a table. The practitioner touches your body quietly. Something changes, but you are not sure what happened. There is no obvious pressing or cracking like a massage or chiropractic session.
Today, I will try to explain this as clearly as I can.
A Licensed Massage Therapist Who Stopped Pressing on Muscles
I hold a national license in massage therapy (Anma Massage Shiatsu) here in Japan. I was trained to press, knead, and release muscles.
But after more than 25 years of clinical practice, I almost never press on muscles anymore.
Why?
Because in most cases, muscle tension is a result, not a cause.
A muscle becomes tight for a reason. For example, tension in a deep membrane called the dura mater can send abnormal signals through the peripheral nerves. The muscles tighten as a response to those signals.
Massage addresses the fact that a muscle is hard. That is not wrong.
But osteopathy asks a different question: “Why is this muscle tight? Why does it need to stay tight?”
This is the most important difference.
Why I Sometimes Choose Not to Release a Tight Muscle
For example, a patient comes in with tightness in the scalene muscles on the side of the neck.
A massage approach would say: the muscle is tight, so let’s release it.
But I often choose not to do that.
Why? Because that muscle is tight for a reason. Something else in the body is out of balance. The scalene muscle is working hard to hold things together. If I release it without addressing the real cause, the body may lose even more balance.
Think of a house that is slightly tilted. One pillar is leaning at an angle. If you straighten that pillar because it looks wrong, the house tilts even more. The pillar was leaning on purpose — to counteract the tilt.
The body works the same way. A tight muscle has a reason for being tight. We need to find and address that reason first.
Why We Look at the Pelvis and Cranium — Not Just the Neck or Lower Back
The most common areas for pain are the neck and lower back. This is not a coincidence.
The lower back is where the pelvis meets the spine. The neck is where the ribcage meets the cranium. Both are junction points — places where large structures connect. Mechanical stress concentrates at these points.
In other words, the neck and lower back are the most vulnerable parts of the body.
This is why osteopathy looks beyond the painful area. Instead of focusing on the neck or lower back, we pay attention to the large structures above and below — the pelvis, the ribcage, and the cranium.
When we find and release accumulated forces in these large structures, the excessive load on the junction points decreases. The body begins to rebalance itself.
Removing Invisible Accumulated Forces
Daily posture. Old injuries. Surgical scars. Childbirth. Years of physical or emotional stress. All of these slowly accumulate forces in the body’s structure.
You cannot see these forces. They do not appear on X-rays or blood tests. But they are there — stored in bones, membranes, fascia, and the tissues that surround the organs.
The body has a remarkable ability to adapt. It compensates for accumulated forces by shifting balance to other areas. This is why you may feel fine for years, even when forces are building up.
But adaptation has limits. When those limits are reached, symptoms appear.
“My back suddenly went out.” “My shoulder froze for no reason.” In most cases, the forces had been accumulating for years. The last small stress simply pushed the body past its limit.
When Forces Are Removed, the Body Recovers on Its Own
The goal of osteopathy is not to “fix” pain.
Our goal is to remove accumulated forces and create an environment where the body’s own healing ability can work properly.
Think of the house again. Remove the forces pulling on the foundation, and the door closes smoothly on its own. You do not even need to repair the door.
The body is the same. When structural forces are released, muscles relax on their own. Blood flow improves. The nervous system settles. The body recovers — because the environment allows it to.
After more than 25 years of working with the body’s structure through my hands, I can say this with confidence.

What is the difference between osteopathy and massage?
Massage releases tight muscles directly. Osteopathy asks why the muscles are tight in the first place and addresses the underlying structural forces that cause the tension. Muscle tightness is usually a result, not the root cause.
Why does my osteopath not always treat the area where I feel pain?
Pain often appears at junction points like the neck and lower back, where large body structures connect. Osteopathy focuses on releasing accumulated forces in the larger structures — the pelvis, ribcage, and cranium — which reduces excess load on these vulnerable areas.
Why does the relief from massage not last?
Massage relaxes the muscles, but the structural forces causing the tightness remain unchanged. When those underlying forces are addressed through osteopathy, the muscles can relax on their own and the relief tends to be more lasting.
